Please review the American Academy of Pediatics policy on care for the male genitals. The basic idea is do nothing to the penis you wouldn't do to eyelids. Clean externally only with clear water. The foreskin is not to be rolled back. If the child is urinating (even if it causes the foreskin to balloon up a bit) there is nothing wrong with having a tight foreskin; it most often resolves by puberty.
